What is Grand Traverse Pavilions?
Grand Traverse Pavilions offers a varied-age constituency a broad range of health care and residential programs and services. As parent organization, The Pavilions has five primary components: Grand Traverse Medical Care, Intergenerational Community Center, Grand Traverse Pavilions Foundation, The Cottages: Adult Residential Community; and the Aquatic/Wellness Center. The Pavilions is the region's only nonprofit, intergenerational continuum of care community. Programs and services range from adult day and overnight respite, to child care, long-term skilled nursing, therapy services, and Alzheimer’s and related dementia care, to supported and assisted living.
Who owns and operates Grand Traverse Pavilions?
Grand Traverse Pavilions is owned by Grand Traverse County and operated by the Grand Traverse County Department of Human Services Board.
Does Grand Traverse Pavilions accept residents from other areas besides Grand Traverse County?
Yes. Adult Day and Overnight Respite Services, Assisted Living, Child Care Services, and The Cottages, Adult Residential Community have no residency restrictions, to date. Because of the waiting list for Grand Traverse Medical Care, there is priority given to residents of Grand Traverse County. We have, however, had residents from adjacent counties in Grand Traverse Medical Care.
Where is Grand Traverse Pavilions located?
The Pavilions is situated on historic Grand Traverse Commons, right in the heart of Traverse City. It is within walking distance of regional cultural, medical, recreational, and retail centers. As a destination, the community is approximately a four-hour drive north of Detroit, two and one-half hours from Grand Rapids, and two and one-half hours south of the Upper Peninsula.
Is The Pavilions affiliated with Munson Medical Center?
No. The Pavilions is situated in close proximity to Munson Medical Center and is easily accessible by means of a connected underground tunnel.
Is any financial assistance available for any of the programs and services at The Pavilions?
Through the Grand Traverse Pavilions Foundation there are grants available to families and individuals interested in Adult Day and Overnight Respite Services. Family Independence Agency vouchers are also accepted for Child Care Services. A matching grant entitled the Snyder Fund is intended to provide a sliding-scale fee for residency in The Cottages, Adult Residential Community.
Is transportation available to residents of Grand Traverse Pavilions?
Grand Traverse Pavilions shuttle busses and drivers provide residents with transportation BATA (Bay Area Transport Authority) also makes regular stops on campus
What social and recreational programs are available to residents of The Pavilions?
Social Services staff offers a range of activities for residents, including daily age-exchanges with the children enrolled in Child Care Services. Volunteers offer residents a range of opportunities, including music, visitation, poetry readings, garden therapy, and special ministries.
Can I bring medications from home to The Pavilions?
No. Residents must get all medications from the pharmacy that The Pavilions contracts with. This is a Public Health guideline. The pharmacy bill will be separate.
Can I bring furnishings from home?
Yes. However, they must fit within the dimensions of the room and/or suite and not hamper resident care or movement.
Can my pets live with me or visit me?
Yes. Within Grand Traverse Pavilions main structure pets are welcome to visit with friends or family. Within The Cottages, pets up to 35 pounds may reside with residents within apartments.
Will my doctor come to The Pavilions to see me?
Yes. If he has admitting privileges. All residents have their own primary care physician. Public Health mandates that they come once every 30 days for the first two months. Then, once every 60 days, thereafter.
Does HIPPA require any notices or information on our website?
Yes. A covered entity that maintains a website describing the services and benefits it offers must make its Notice of Privacy Practices prominently available through the site and make the notice available electronically through the website. See our Joint Notice of Privacy Practices.

What is Grand Traverse Medical Care?
Grand Traverse Medical Care (GTMC) is the flagship operation of Grand Traverse Pavilions. Established in 1959, GTMC provides long-term skilled nursing care to residents 18 years and older. A full continuum of Alzheimer’s and related dementia care along with rehabilitative therapy services is also included within the program, which is recognized among the top third long-term care operatives in the country.
What reimbursement is available for Grand Traverse Medical Care residents?
Medicare will only pay when there is a three (3) day hospital stay prior to admission to the nursing home and the resident meets the requirements for skilled care. Medicaid is available and private pay insurance.
What services does Medicare pay for in a nursing home?
An individual’s care needs must meet the skilled nursing care criteria. Otherwise, Medicare and any secondary insurance will not pay. Secondary insurance (i.e. Blue Cross) is only a compliment to Medicare. It only pays if Medicare pays first.

What is Adult Day Services?
Our program at The Pavilions is two-fold. First, Adult Day Services offers a safe environment with supervised activities that address a participant’s social, physical, emotional, nutritional, spiritual and basic health needs. The second purpose is intended to provide respite to care givers, affording them peace of mind and the support they need to keep a loved one living at home.
Respite Services. What does that mean?
Individuals who qualify for Assisted Living are eligible for Overnight Respite Services. The Pavilions offers a furnished suite within historic Willow Cottage that can comfortably accommodate an individual. The suite is available seven days a week by advance registration for stays up to several weeks.

What does Assisted Living mean?
Assisted Living at Grand Traverse Pavilions gives residents the opportunity to not only live life independently, but to get support with daily tasks and personal care. Accommodations for Assisted Living tenants are within historic Willow Cottage as well as Maple Pavilion in the main building of The Pavilions. There are 23 suites at Willow and 12 at Maple. Maple specifically offers dementia care to individuals in need of assisted living.

Assisted Living tenants have access to the full continuum of care of The Pavilions. Daily age exchanges are a vital component of the socio-recreational programming.

What type of person is eligible for Assisted Living?
An Assisted Living resident at The Pavilions is someone who is 18 years or older; ambulatory; continent; able to communicate wants and needs; and does not require continuous skilled nursing care.
Is your Assisted Living program licensed?
No. The State of Michigan does not currently license Assisted Living providers.
Does Medicare or Medicaid pay for assisted living?
There is currently no government reimbursement for Assisted Living. However, there may be the potential for some financial assistance through Medicaid’s Waiver Program in the future.
What happens if a resident in Assisted Living requires more care than can be provided for in Assisted Living?
Residents who participate in any of the programs offered at The Pavilions automatically have access to our entire Continuum of Care, which includes skilled nursing services. Our staff works with residents and families to assure individual care needs are met in the appropriate setting.

Who is eligible for Child Care Services at The Pavilions?
The Intergenerational Community Center at Grand Traverse Pavilions offers licensed Child Care Services to both our employees and residents from the community. Infants, toddlers, and preschoolers are eligible for daily care.

What is the Grand Traverse Pavilions Foundation
Founded in 1997, the Foundation at Grand Traverse Pavilions oversees the development and fund raising initiatives of The Pavilions. The Foundation strives to ensure that all residents and guests of The Pavilions have access to the full range of programs and services provided by The Pavilions.
Does the Foundation accept donations or bequests from wills and trusts?
Yes. Grand Traverse Pavilions Foundation offers a variety of giving vehicles, including planned giving, memorial gifts and trusts, and donations to annual and corporate campaigns.

What are The Cottages?
The Cottages are the second phase of development for Grand Traverse Pavilions. An adult residential community, they offer approximately 76 studio and one bedroom apartments with a variety of hospitality services.
Is there an age restriction of residency in The Cottages apartments?
The Cottages offer single and double occupancy to adults age 18 and older. Grand Traverse Medical Care, the skilled nursing operative of The Pavilions, is open to individuals 15 years and older.
Are The Cottages just for people who need Assisted Living?
No. Our residential community is open to adults who have varying levels of needs - from independent to supported living.
Do you have to purchase an apartment or sign an annual contract?
No. Month-to-month rentals are available. A one-time-only application fee is required. The first month’s rent and a deposit of the last month’s rent are needed upon the monthly residency contract signature.
Are the apartments in The Cottages furnished?
Each unit comes with carpeting, window treatments, cabinetry in the kitchenette and bathroom, microwave, and under-counter refrigerator. Furniture must be provided by the lessee. Limited storage space is offered. Hook-ups are available for phones and cable.
What’s included in The Cottages monthly rental?
All basic utilities, three family-style meals daily, weekly housekeeping, weekly linen and towel service, weekly personal laundry service, social and recreational programming, scheduled transportation, building and grounds maintenance, 24-hour staffing, 24-hour emergency response system, access to The Pavilions continuum of care, and pets on the Terrace Level.

What impact does the Aquatic/Wellness Center have on constituents?

As an on-site environment, the Center offers constituents the opportunity to exercise in safe dry land and aquatic environments with trained professionals in order to maximize function. Rehabilitative therapy service, including physical, occupational, recreational, and aquatic therapy as well as speech-language pathology and restorative nursing are also accessible.

Is the Aquatic/Wellness Center open to the community?

Yes, the public may access rehabilitative therapy services, including physical, occupational, and aquatic therapy as well as speech-language pathology. In addition, the Center offers aquatic exercise classes to meet various fitness levels.

Who provides Center services?

Grand Traverse Pavilions has its own in-house team of professional therapists. There are physical therapists and physical therapist assistants, occupational therapists and certified occupational therapy assistants, speech-language pathologists, aqua therapist, recreation therapists, and restorative nursing assistants.

Do you need a physician's prescription to participate?

Yes, you need physician's orders for rehabilitative therapy services. It is recommended that you consult with your physician before starting any exercise program.

Who pays for Center rehabilitative therapy ?

Constituents of The Pavilions who require rehabilitative therapy services may be covered by his/her Medicare insurance. Other private insurances may or may not cover therapy.

Who pays for exercise classes?

The individual is responsible for each class.

Do I need to know how to swim to participate in aquatic exercise classes?

No. Classes are conducted in a maximum of four feet of water. The individual does not submerge during class. An instructor is required to be in attendance.
